Reporting incidents and near misses helps everyone learn from each event and take steps to prevent future injuries and accidents. It is highly encouraged to report any and all incidents and near misses no matter how small through ARISE.
Serious Incident
A serious incident has occurred in any of the following situations:
- Injury or incident that results in the death of a worker
- Injury or incident that results in worker admitted to hospital
- Unplanned or uncontrolled explosion, fire, or floods that causes (or had the potential to cause) a serious injury
- Collapse or failure of any part of a building or structure that is necessary for its structural integrity
- Collapse of a hoist, crane, or derrick
To report a serious incident you must:
- Call the Control Centre: 780-492-5555
- Report your incident, provide your phone number, and request a call back from the HSE on-call person
- Complete and submit an incident report
- If there was a injury you must fill out a Workers’ Compensation Board (WCB) report within 48 hours of the accident
- Notify the EAS Safety Committee of the incident
Serious incidents are also reported to the Office of Occupational Health and Safety (OHS) for investigation. HSE will contact them on the behalf of the reporter and advise with any interactions and reports.
Incidents and Near Misses
Any situation that resulted or could have resulted with injury or property damage that does not qualify as a serious incident, but should still be reported
Reporting Incidents and Near Misses:
- Complete and submit an incident report through ARISE
- Your supervisor will then be notified to complete the second part of the form
- If there was a injury you must fill out a Workers’ Compensation Board (WCB) report within 48 hours of the accident
- Notify the EAS Safety Committee of the incident
